Printing and filling out these forms ahead of time helps reduce wait time and allows our team to focus on welcoming your child and answering any questions you may have.

- Email them to our office prior to your visit, if preferred

A Gentle Reminder
If your child is under 24 months of age and coming for a complimentary first visit, we still ask that forms be completed in advance so we can best understand your child’s medical and dental history.
